Supplies:

Tube of your choice. I am using the artwork of Jeff Pittarelli. I got this image when he was with CILM. I do not know where to purchase his tubes now.

Scrap kit is called Autumn Delight by MizTeeque. You can get this kit at http://mizteeques.blogspot.com/

Brushes of your choice. You can get some great ones at deviantart.com

Font of your choice.

Lets Begin:

Open element 40 and resize by 85%. Copy and paste as a new layer in the center of a new canvas that gives you a lot of room to work with.
Using your freehand selection tool set on point to point select around the outside edges of the frame.
Add a new raster layer and drag under the frame.
Open paper 8 and copy and paste into selection.
Selections select none.
Using your freehand tool set on point to point still select the inside part of the frame this time.
Open your tube and copy and paste as a new layer and position in the frame.
Selections invert and hit delete.
Add a drop shadow to both the frame and your tube.
Open element 3 and resize.
Copy and paste as a new layer and move to the bottom right of the tag.
Duplicate and image flip horizontal. Move this duplicate to left hand side of the tag.
Add a drop shadow to both and merge together once in place.
Open element 6 and and repeat the steps as above.
Open element 17 and resize. Copy and paste as a new layer and go to image rotate to the left 48 degrees.
Adjust sharpen.
Place in the center at the bottom.
Open element 23 and copy and paste as a new layer and place on the bottom right.
Open element 29 and resize and copy and paste as a new layer. Drag under the flower. Move to the right of it.
Open element 50 and resize and copy and paste as a new layer and move to the bottom right in front of the flower.
Make your gradient layer active. Go to selections select all float and defloat.
Add a new raster layer.
Apply some brushes so that it covers all the empty space around your tube.
I also added some text to the top but that is up to you.
Add your name and copyright information.
Crop and save.
