Supplies Needed:

Tube of your choice. I am using the work of Anna Marine. You can get this tube at http://cdoestore.com

Font of your choice. I am using Avquest and Arlekino.

Lets Begin:
Open element 29 and shift +d to duplicate.
Close the original.
Resize by 85%.
Make your canvas large enough to work with and center this element in the canvas if you need to.
Add a nice drop shadow to darken it up a bit.
Open element 54 and resize so it will fit nicely into the top left of the tag.
Select the inside of the frame with your magic wand and go to selections modify and expand by 3.
Add a new raster layer and drag under the frame.
Flood fill with a grey gradient.
Open your tube and resize so just the face will fit in the frame and position.
Go to selections invert and hit delete.
Selections select none.
Change the properties of the layer to luminance legacy.
Lower the opacity of the this layer to 60.
Add a drop shadow to the frame.
Open your tube and resize if you need to.
Copy and paste as a new layer and position on the right side of your tag.
Add a drop shadow.
Open element 3 and resize by 75% 3x.
Rotate to the left by 25 degrees 2x. Don't do 50 degrees 1 time because it makes it off so just do the 2 steps.
Copy and paste as a new layer and move to the far left bottom.
Duplicate and on the duplicate flip horizontal.
Move to the far right side bottom.
Merge these 2 layers together and add a drop shadow.
Open element 7 and rotate to the right.
Copy and paste as a new layer and paste on top of the black ones.
Add a drop shadow.
Open element 6 and resize so it is rather small and copy and paste as a new layer and place down amongst the flowers.
Duplicate several times and scatter them.
Repeat the above steps with element 5.
Open element 45 and copy and paste as a new layer and place on the far left of the tag and drag so its under the flowers and the framed image.
Open element 58 and resize by 50% and copy and paste as a new layer and place on the far right side of the tag.
Open element 57 and resize by 50% and copy and paste as a new layer and move to the left but to the right of the cross.
Image rotate to the left 25 degrees.
Open element 33 and resize by 50%. Copy and paste as a new layer and place on the stem of the wine goblet.
Using a font of your choice add some text to your tag.
Add your name.
Add your copyright information.
Crop and save.
